How to complete the online direct debit form.

Below is a screenshot of the online direct debit form for upfront payments. The fields mark with an asterisks (*) are mandatory and self-explanatory. The highlighted text in some fields explains how to complete those fields. Note: the date fields cannot be locked by us. You must enter the dates as per highlighted instructions. If you do not complete the form as per instructions, we will correct it manually.
